Role: Registered Nurse, Hospice Case Manager (RN)

Overview:

This is an opportunity for a dedicated Registered Nurse Case Manager to join our client and make a difference in the lives of patients in hospice care. You will be part of a supportive team, providing essential care and coordination for patients and their families. 
Key Responsibilities: 
● Manage patient care in hospice settings. 
● Coordinate with healthcare teams.
● Provide leadership and support to nursing staff.
Schedule and Shift Details Schedule: Monday – Friday 8:00 am – 6:00 pm, 1 weekend per month, and 2 holidays per year Travel 
● Local travel required covering multiple counties.
